Special Provision for Disconnection of Package
If a customer is disconnected from a package due to nonpayment, the customer can subscribe to basic, flat-rate access line service by paying an amount equal to the rate for the basic, flat-rate access line service, plus applicable fee, taxes and surcharges, for the period of nonpayment up to date of disconnection, as adjusted for partial payments.
Letter of Agency
This form gives Lightcurve your permission to switch your long distance phone service (local toll, state-to-state, and international calling) over to Lightcurve on your behalf.
Opt-out of CPNI
This form tells Lightcurve not to use information about your phone service usage (like call details from your bill) to market additional products and services to you.
Permission for Inside Wiring
This form gives Lightcurve your permission to do the physical installation work at your address, like running a line from the street and drilling for inside wiring.
Service Installation Agreement
This form asks you to review and accept the terms of your service installation, including equipment, wiring, promotions, and cancellation charges.
