Grant of Agency Authority

This letter authorizes Rainier Connect, consistent with FCC requirements to act as our agent to change our primary inter-exchange carrier for each of the telephone numbers listed on this form.

We understand that only one inter-exchange carrier may be designated as our interstate primary interexchange carrier (PIC) for any one telephone number and we further understand that any PIC change made on our behalf may involve the imposition of a charge by a local exchange carrier that we are responsible for paying.

Rainier Connect, is further authorized to change our primary carrier for intrastate (intraLATA) and international calling services for each of the telephone numbers listed on this form or any supplement to this authorization.

We understand that the terms, conditions and rates for Rainier Connect’s long distance services are found at www.rainierconnect.com (or from the business office) and that use of any of the services constitutes our agreement to the terms, conditions and rates that apply to that service.

*Rainier Connect Unlimited Long Distance: Requires Rainier Connect local service. Unlimited Long Distance within the contiguous US. Cannot be used for business or Internet access. Taxes, surcharges, and regulatory fees not included. Usage may be monitored and customer may be required to show compliance if usage exceeds 3,000 minutes/mo. or non-compliance indicated. All rates subject to change.
